Where is it possible to obtain this form?

It is possible that you had already obtained this formulary together with the "green card" while sighing the contract about damage liability (it is self-copying).

From 1st January 2009 it is a duty to carry the formulary in your car. From January 2009 the participants of the motor
vehicle accident can call the police only to some accidents. If the participants do not have the duty to call the police they
have to document the traffic accident as attentively as possible. In case that they do not do so or they do not prove the
evidence correctly, the insurance company would not pay the insurance benefit.

The form about the motor vehicle accident fully corresponds with the model created by Comité Européen des Assurances (CEA). The schedule of this formulary and the sequence of the cells is consistent with forms in other languages, so at the accident with foreigner each participant of the accident fills the form in his/her langugage. It is necessity to fill the form twice, each participant will obtain one copy. In case that you do not have the formulary use any paper you have but the text must contain all important points. The law defines that "the record has to contain the identification of the place and time of the accident, the participants and motor vehicles, the accident´s cause, course and consequences".
